Psychological Stress and Cognitive Aging in Older Adults
Summary
Psychological stress represents an array of emotional and physiological responses to perceived challenges or threats, and its chronic activation has been implicated in the acceleration of cognitive ageing. In older adults, persistent stress may dysregulate the hypothalamic–pituitary–adrenal axis and promote inflammatory pathways, leading to neuronal vulnerability in regions subserving memory, executive function and processing speed. Life-course exposures—ranging from early adversity to daily hassles in later life—can accumulate to influence both baseline cognitive performance and trajectories of decline. Conversely, individual differences in psychological resilience, coping strategies and lifestyle factors such as physical activity and social engagement may buffer the impact of stress on brain health. Understanding the multifaceted interplay between stressors, biological mediators and protective mechanisms is crucial for informing interventions to preserve cognitive function and reduce dementia risk in ageing populations.

Recent studies have identified a common genetic variant in a stress-related gene that confers a heightened risk of all-cause dementia, potentially via altered DNA methylation and increased tau pathology in the ageing brain. In parallel, a multi-centre longitudinal investigation of individuals with amnestic mild cognitive impairment revealed that standard questionnaires of recent life events and perceived stress were not independently predictive of subsequent cognitive decline or conversion to dementia, whereas elevated salivary cortisol levels correlated with poorer baseline cognition and influenced the pace of change over follow-up.

Technical terms
Amnestic mild cognitive impairment (aMCI): A clinical syndrome characterised by isolated memory deficits that exceed age-expected decline but do not yet fulfil criteria for dementia.
Hypothalamic–pituitary–adrenal (HPA) axis: The central neuroendocrine system that regulates the stress response through release of corticotropin-releasing hormone, adrenocorticotropic hormone and cortisol.
DNA methylation: An epigenetic modification involving the addition of methyl groups to cytosine bases in DNA, affecting gene expression without altering the underlying sequence.
Perceived Stress Scale (PSS): A self-report instrument measuring the degree to which situations in one’s life are appraised as unpredictable, uncontrollable and overloaded.
Tau pathology: The abnormal aggregation and hyperphosphorylation of tau proteins in neurons, contributing to neurofibrillary tangles and neurodegeneration.
