Psychotherapeutic Approaches for Adolescent Depression

Summary

Adolescent depression presents a global public health challenge, with onset often coinciding with critical developmental milestones. Psychotherapeutic strategies have evolved from disorder-specific protocols towards flexible, personalised and scalable models. Traditional approaches such as cognitive behavioural therapy (CBT) and interpersonal psychotherapy (IPT) remain first-line treatments, emphasising cognitive restructuring, behavioural activation and the resolution of interpersonal stressors. Short-term psychoanalytic and psychodynamic methods aim to enhance reflective functioning and emotional insight. More recently, transdiagnostic interventions target core processes—such as emotion regulation, rumination and avoidance—common to mood and anxiety disorders, facilitating broader applicability in routine settings. Family-based and parental involvement components are increasingly incorporated to address environmental and systemic factors. Parallel advances in digital therapeutics and school-based group programmes seek to extend access, reduce stigma and lower cost. Collectively, these approaches demonstrate moderate efficacy in symptom reduction and functional recovery, while also highlighting unmet needs in treatment non-response, maintenance of gains and equitable delivery across diverse populations.

Psychotherapeutic Approaches for Adolescent Depression publication trend

The graph below shows the total number of articles in psychotherapeutic approaches for adolescent depression across all publications each year (not limited to Nature Index journals).

Technical terms

Cognitive behavioural therapy (CBT): A structured, time-limited therapy that addresses maladaptive thoughts and behaviours to alleviate depressive symptoms.

Interpersonal psychotherapy (IPT): A brief, focused treatment targeting interpersonal disputes, role transitions and social deficits that contribute to depression.

Transdiagnostic intervention: A therapeutic approach designed to treat shared underlying mechanisms across multiple disorders, enhancing flexibility and broad applicability.

Behavioural activation: A core CBT technique that increases engagement in positively reinforcing activities to counteract avoidance and low mood.

Digital therapeutics (DTx): Software-driven interventions, often delivered via mobile apps, that provide evidence-based therapeutic content with minimal therapist contact.

Remission: A clinically meaningful reduction of depressive symptoms to below diagnostic thresholds, often a primary goal of intervention studies.
