Summary
Individuals living with HIV face a disproportionately high burden of trauma and post-traumatic stress disorder (PTSD), arising from factors that include violence, stigma, bereavement and the profound shock of diagnosis. These psychological sequelae can compromise antiretroviral therapy adherence, undermine social support and exacerbate comorbid conditions such as depression and substance misuse. In recognition of these challenges, recent efforts have centred on integrating mental health services into HIV care pathways, adopting trauma-informed approaches, and evaluating both pharmacological and psychotherapeutic interventions. Cognitive behavioural therapy, peer support groups, and task-sharing within primary care settings have shown promise in fostering resilience, reducing PTSD symptoms and enhancing adherence to treatment. Technological innovations, including digital screening tools and tele-counselling, are extending reach into marginalised communities. Globally, there is an emerging consensus that mental health interventions must be culturally adapted, delivered in decentralised settings and underpinned by sustained training of healthcare providers to address the complex interplay between trauma, stigma and living with HIV.

Post-traumatic stress disorder (PTSD): A psychiatric condition triggered by exposure to actual or threatened death, serious injury or sexual violence, characterised by intrusive memories, avoidance behaviours, negative mood and hyperarousal.
Antiretroviral therapy (ART): The regimen of antiviral medications used to suppress HIV replication, improve immune function and reduce morbidity and mortality among people living with HIV.
ART adherence: The extent to which a person’s behaviour in taking medication corresponds with agreed recommendations from a healthcare provider; crucial for virological suppression.
Trauma-informed care: An approach that recognises the prevalence and impact of trauma, emphasises physical, psychological and emotional safety, and seeks to empower patients in their treatment.
Cognitive behavioural therapy (CBT): A structured, time-limited psychotherapeutic approach that addresses maladaptive thought patterns and behaviours to alleviate psychological distress.
