Summary
Pulsed radiofrequency (PRF) has emerged as a non-destructive neuromodulation technique for a range of chronic pain conditions. By delivering short bursts of high-frequency electrical energy through a specialised electrode, PRF generates an electromagnetic field that modulates nociceptive signalling without creating the thermal lesion typical of continuous radiofrequency ablation. This modulation appears to reduce pro-inflammatory cytokine levels, alter ion channel function and disrupt pain transmission in small sensory fibres, offering relief while preserving surrounding tissue integrity. Clinically, PRF can be applied percutaneously to target dorsal root ganglia, peripheral nerves and epidural spaces, or non-invasively via transcutaneous delivery. Its versatility has led to combinations with steroid injections and orthobiologic agents, aiming to synergise immunomodulation with endogenous repair pathways. Ongoing animal studies and human case series have begun to elucidate the underlying electromagnetic-membrane interactions and anti-inflammatory effects, supporting its global significance as an adjunct or alternative to pharmacotherapy and surgical intervention in conditions such as radicular back pain, neuropathic syndromes and joint-related discomfort. The practical applications extend from office-based procedures to ambulatory care, aligning with the unmet need for minimally invasive, targeted therapies in the face of a growing chronic pain burden worldwide.
