Summary

Rapid initiation of antiretroviral therapy (ART) following an HIV diagnosis has emerged as a critical strategy to curtail viral replication, reduce morbidity and mortality, and prevent onward transmission. Traditionally, ART commencement involved multiple clinic visits for laboratory assessments and counselling, often leading to delays and loss from the care cascade. The move towards same-day or rapid ART initiation seeks to simplify this process, fostering higher uptake, earlier viral suppression and improved long-term retention in care. Practical applications have spanned diverse settings—from urban clinics in sub-Saharan Africa to community-based testing campaigns—highlighting the role of point-of-care diagnostics, streamlined counselling and integrase inhibitor–based regimens in achieving prompt treatment. This approach aligns with global goals to reach viral suppression targets, optimise individual health outcomes and reduce the public health burden of HIV.

Observational findings demonstrate that initiating ART at the first post-diagnosis visit can lead to exceptionally rapid viral load reduction. In one analysis, newly diagnosed individuals beginning treatment within thirty days achieved a median eight-day interval from regimen offer to initiation. Those on integrase inhibitor–based regimens reached viral suppression in a shorter timeframe than on protease inhibitor–based alternatives, with suppression rates exceeding 80 per cent by week 24 and 88 per cent by week 48. These data underscore the feasibility and efficacy of very early ART initiation in both acute and chronic infection phases when supported by streamlined clinical pathways.
