Summary

REM sleep behavior disorder (RBD) is characterised by the loss of normal muscle atonia during rapid eye movement sleep, leading to dream enactment and potentially injurious movements. In Parkinson’s disease (PD), RBD is one of the most prevalent non-motor manifestations, affecting up to half of patients over the course of illness. Its onset often precedes the classic motor signs of PD by several years, reflecting early involvement of brainstem nuclei and alpha-synuclein pathology. Clinically, RBD in PD is associated with more rapid progression of motor symptoms, greater burden of cognitive impairment and higher likelihood of developing dementia. Neuroimaging studies have revealed atrophy in brainstem regions that regulate muscle tone, alongside wider subcortical and cortical changes. The presence of RBD thus serves as both a prodromal marker and a predictor of a more aggressive disease phenotype, highlighting the need for early detection and targeted management strategies.

Analyses of large MRI datasets have demonstrated that PD patients with RBD exhibit pronounced volume loss within the pontomesencephalic tegmentum, hypothalamus, thalamus, basal ganglia and limbic structures. These patterns of atrophy implicate cholinergic and GABAergic circuits in the failure of REM-related muscle inhibition and underscore the shared neurodegenerative substrate driving both sleep and motor dysfunction. Complementary meta-analyses of clinical cohorts have identified distinct demographic and disease characteristics in PD with confirmed versus probable RBD. Patients with definite RBD tend to be older, more often male, and exhibit longer disease duration, higher motor scores and more advanced Hoehn–Yahr stages. Probable RBD, assessed without polysomnographic confirmation, similarly correlates with increased motor severity and suggests a continuum of clinical risk that can be captured by screening questionnaires in routine practice.
