Summary
Renal fusion anomalies comprise congenital disorders in which one or both kidneys are abnormally positioned or fused during embryogenesis. The most common types are horseshoe kidney and crossed fused renal ectopia. In a horseshoe kidney the lower poles of the two kidneys join across the midline, whilst in crossed fused ectopia the second kidney crosses to the opposite side and fuses with its partner. These anomalies often remain undetected until imaging is undertaken for unrelated conditions, yet they may predispose to urinary tract infection, urolithiasis, hydronephrosis and vesicoureteral reflux. Anomalous vasculature, ureteral malrotation and aberrant collecting systems add complexity to any interventional procedure. Contemporary management begins with accurate delineation by ultrasonography, CT, MRI or radionuclide scanning. Asymptomatic patients may be managed conservatively with regular monitoring, whilst symptomatic cases demand tailored interventions. Minimally invasive endourological techniques, including percutaneous nephrolithotomy and extracorporeal shock wave lithotripsy, have gained prominence for stone removal, and reconstructive surgeries such as pyeloplasty or ureteric reimplantation can preserve renal function when obstruction or reflux is present. Multidisciplinary collaboration ensures optimum outcomes.
