Summary

Root canal morphology encompasses the intricate three-dimensional architecture of the pulp chamber and root canal system within teeth. Variations in canal number, curvature, ramification and connectivity present significant challenges for debridement and obturation. Molars typically display multiple roots and canals, premolars show regional diversity and anterior teeth may harbour unexpected bifurcations or C-shaped systems. Successful endodontic therapy depends on accurate preoperative mapping of this anatomy to guide access, instrumentation and irrigation. Traditional periapical radiography offers two-dimensional projections but is prone to superimposition and limited in detecting fine canal details or complex curvatures. Cone Beam Computed Tomography (CBCT) has emerged as the clinical standard for three-dimensional visualisation with adjustable field of view and relatively low radiation dose. Research-grade micro-computed tomography (micro-CT) provides sub-millimetre resolution for ex vivo studies, elucidating morphological patterns and informing classification schemes. Developments in software enable automated segmentation, statistical shape modelling and real-time guidance. The integration of advanced imaging with nickel-titanium rotary instruments, ultrasonics and irrigation activation techniques enhances the predictability of canal negotiation and disinfection. Globally, these advances support minimally invasive access designs, reduce procedural errors and improve long-term outcomes in diverse patient populations.

Research from Nature Portfolio

A large-scale ex vivo study of third molar teeth analysed nearly 1,200 specimens to chart root and canal variations beyond established classification systems. Novel configurations were documented in both maxillary and mandibular third molars, including extra roots, lateral canals and inter-canal communications not previously described. Statistical analysis revealed that nearly 70 % of maxillary third molars had three roots and over half of mandibular third molars exhibited three canals, underscoring the unpredictable anatomy of wisdom teeth. The work highlights the need for personalised preoperative imaging and suggests that bespoke treatment strategies based on detailed morphological mapping can reduce intraoperative complications.

Micro-computed tomography (micro-CT): High-resolution ex vivo X-ray imaging technique that generates detailed three-dimensional reconstructions of dental tissues at the micrometre scale.

Vertucci’s classification: A widely used system categorising canal configurations into eight types based on the number of canals and their pathways from pulp chamber to root apex.

C-shaped canal: A morphological variant in which the root canal cross section forms a continuous C-shaped channel, often found in mandibular second molars.
