Summary

Salivary metabolomics entails the large-scale identification and quantification of small molecules in saliva, offering a non-invasive window into oral physiology and pathology. Advances in nuclear magnetic resonance spectroscopy and mass spectrometry have enabled the characterisation of diverse compound classes—including amino acids, organic acids, saccharides, polyamines and lipid mediators—reflecting both host and microbial activity. In oral health, baseline salivary metabolic profiles support functions such as taste perception, mucosal integrity and antimicrobial defence. In disease states, perturbations in metabolites mirror microbial dysbiosis, inflammatory cascades and tissue breakdown. Caries, periodontitis, oral cancers and systemic conditions with oral manifestations each exhibit distinctive metabolic signatures. The global significance of this research lies in its potential to deliver rapid, chair-side diagnostics, personalised risk assessment and real-time monitoring of treatment efficacy. Integration of salivary metabolomics with clinical indices and microbiome data promises a holistic approach to predictive, preventive and personalised oral healthcare.

Recent studies have applied salivary metabolomics to define biomarkers of periodontal inflammation. One investigation profiled whole-mouth saliva before and after supragingival debridement in subjects with varying degrees of periodontal inflamed surface area. Multivariate analyses identified cadaverine and hydrocinnamate as markers positively correlated with disease severity, while uric acid and ethanolamine were more abundant in health. Metabolite set enrichment revealed that pathways of polyamine metabolism, arginine and proline degradation, butyric acid metabolism and lysine catabolism were enriched in individuals with advanced periodontal inflammation, underscoring the metabolic contributions of dysbiotic plaque communities. These findings validate salivary metabolites as robust indicators of periodontal status and offer mechanistic insight into microbe-host interactions.
