Summary

Sarcomatoid liver tumours represent a rare and aggressive subset of primary hepatic malignancies, exhibiting a biphasic histology that combines epithelial carcinomatous elements with mesenchymal sarcomatoid components. These tumours often present in the setting of chronic liver disease and display rapid growth, extensive necrosis and a propensity for vascular invasion and early metastasis. Imaging plays a central role in detection, characterisation and preoperative planning. Computed tomography and magnetic resonance imaging typically reveal large, subcapsular masses with heterogeneous signal intensity, rim-like or progressive enhancement patterns and frequent evidence of capsular breach or satellite nodules. The sarcomatoid component contributes to atypical imaging features, such as cystic degeneration, haemorrhage and heterogeneous perfusion. Accurate preoperative identification requires a combination of multiphase imaging, serological marker assessment and, increasingly, image-guided biopsy targeting viable tumour regions. Improved molecular profiling has begun to unveil distinct genomic alterations that may inform prognosis and guide targeted therapies. Despite advances in diagnostic techniques, overall survival remains poor, underscoring the need for refined diagnostic criteria and novel therapeutic approaches.
