SARS-CoV-2 Transmission Dynamics in Household Settings

Households represent a principal locus for SARS-CoV-2 spread, accounting for a substantial proportion of secondary cases observed during the pandemic. Transmission dynamics within domestic environments are shaped by a complex interplay of viral factors (including variant transmissibility and viral load kinetics), host immunity (natural infection, vaccination and hybrid profiles), behavioural interactions (duration and proximity of contact among family members) and environmental conditions (ventilation, space density and hygiene practices). Secondary attack rates in households have ranged widely, from under 10 per cent in early reports to over 50 per cent for highly transmissible variants, highlighting the effect of evolving viral lineages and waning immunity. Age-related susceptibility and infectivity also diverge, with children initially showing reduced transmission potential but later variants eroding this distinction. Practical interventions—rapid case identification, isolation of index cases from susceptible contacts, enhanced ventilation and consistent mask use—remain critical to curbing domestic spread, with particular relevance in regions where multigenerational living is common or vaccine coverage is incomplete.

Recent studies examining tens of thousands of household index cases and their contacts have clarified how prior immunity shapes SARS-CoV-2 propagation. Analysis of data collected over multiple variant waves demonstrates that immunity primarily lowers the susceptibility of exposed contacts rather than markedly reducing the infectiousness of index cases. Natural infection confers the strongest protective effect against subsequent household acquisition, with hybrid immunity showing no consistent advantage over recent infection alone. Vaccination contributes a modest but more stable reduction in infectiousness than protection against acquisition, even as new variants emerge. These findings reinforce the complementary role of vaccination alongside non-pharmaceutical measures such as mask use and environmental controls in minimising intrafamilial transmission.
