Self-Monitoring of Blood Glucose in Type 2 Diabetes Management

Summary

Self-monitoring of blood glucose (SMBG) is the regular measurement of capillary blood glucose by individuals with type 2 diabetes, typically using a portable glucometer. This practice empowers patients to understand daily glycaemic patterns, adjust lifestyle and dietary behaviours, and optimise pharmacotherapy in collaboration with healthcare professionals. Advances in sensor technology, connectivity and data visualisation have broadened the scope of self-monitoring from intermittent finger-prick testing to semi-continuous and continuous glucose monitoring systems. Structured SMBG protocols—where timing, frequency and interpretation are pre-defined—facilitate pattern recognition and targeted intervention. Globally, SMBG underpins individualised diabetes care plans, supports risk reduction for microvascular and macrovascular complications, and enhances patient engagement in self-management.

Research from Nature Portfolio

Recent observational analyses have demonstrated that early initiation of SMBG in newly diagnosed, non-insulin-treated patients is associated with improved glycaemic control over the first year. Patients who performed baseline SMBG achieved a maximal reduction in glycated haemoglobin of approximately 0.55% at three months compared with non-users, with sustained benefits lasting up to twelve months. Subgroup evaluation revealed consistent gains regardless of whether oral secretagogues or other non-insulin therapies were used. These findings underscore the role of SMBG as an early behavioural intervention that can shape long-term metabolic outcomes in type 2 diabetes.

The graph below shows the total number of articles in self-monitoring of blood glucose in type 2 diabetes management across all publications each year (not limited to Nature Index journals).

Technical terms

Self-Monitoring of Blood Glucose (SMBG): The practice of patients measuring their own blood glucose levels at home, usually via finger-prick sampling and portable glucometers.

Glycated Haemoglobin (HbA1c): A biomarker reflecting average blood glucose concentrations over the preceding two to three months, expressed as a percentage or mmol/mol.

Continuous Glucose Monitoring (CGM): A sensor-based system that provides near-real-time glucose readings and trend data throughout the day and night.

Structured Self-Monitoring: A predefined SMBG regimen specifying the timing, frequency and context of glucose measurements to detect glycaemic patterns and guide therapeutic adjustments.
