Serotonin Toxicity and Clinical Management
Summary
Serotonin toxicity, often termed serotonin syndrome, arises from excessive serotonergic activity within the central nervous system. It manifests as a triad of altered mental state, autonomic dysregulation and neuromuscular hyperexcitability. The condition typically results from polypharmacy involving serotonergic agents such as selective serotonin reuptake inhibitors, serotonin–noradrenaline reuptake inhibitors, certain opioids and monoamine oxidase inhibitors, or from interactions with over-the-counter medications. Clinical presentation ranges from mild tremor and agitation to life-threatening hyperthermia, seizures and multi-organ dysfunction. Prompt recognition relies on clinical criteria and exclusion of mimicking conditions such as neuroleptic malignant syndrome. Management centres on immediate discontinuation of offending agents, supportive measures including intravenous fluids, active cooling and sedation with short-acting benzodiazepines, and, in moderate to severe cases, serotonin-receptor antagonists. Continuous monitoring of vital signs and neuromuscular signs is essential. Growing antidepressant use, expanding indications for serotonergic drugs and increasing polypharmacy underscore the global importance of standardised diagnostic strategies and evidence-based management protocols to reduce morbidity and mortality.
