Summary
Shift work encompasses any employment schedule that falls outside traditional daytime hours. Such schedules disrupt the temporal alignment between endogenous circadian rhythms and external demands, leading to sleep disturbances, excessive daytime sleepiness and impairment of cognitive performance. The misalignment of biological clocks, compounded by irregular shift rotations and insufficient rest, exacerbates cardiometabolic dysregulation, hormonal imbalances and gastrointestinal disturbances. Over time these alterations heighten the risk of obesity, insulin resistance, cardiovascular disease and mental health complaints. Social and familial well-being is also affected, as atypical hours interfere with social and leisure activities, contributing to fatigue and decreasing overall quality of life. Ergonomic approaches to shift planning, including the optimisation of rotation speed, direction of rotation and opportunities for individual adjustment, have emerged as practical strategies to attenuate adverse outcomes. These interventions have particular relevance in safety-critical industries such as healthcare, transport and emergency services, where lapses in alertness can have serious consequences. Recent advances in objective register-based assessments have improved the characterisation of working time patterns, facilitating large-scale epidemiological studies that identify the toxic components of shift schedules.

A controlled cross-over investigation simulated day and night duty to examine the effects of circadian misalignment on cognitive function in chronic shift workers. Under misaligned conditions modelling night shifts, participants exhibited marked decrements in sustained attention, information processing and visual-motor performance, particularly after prolonged wakefulness. Increased subjective sleepiness and reduced sleep efficiency correlated with these performance impairments. The findings demonstrate that circadian disruption inherent to night shifts substantially compromises cognitive resources, highlighting the importance of strategies to promote circadian alignment and sufficient sleep consolidation among long-term shift workers.
