Sleep Disturbances in Children with Attention-Deficit/Hyperactivity Disorder
Summary
Children with attention-deficit/hyperactivity disorder (ADHD) frequently experience a spectrum of sleep disturbances that encompass delayed sleep onset, fragmented sleep, reduced total sleep time and excessive daytime somnolence. Subjective reports often align with objective findings of altered sleep architecture, including prolonged rapid eye movement (REM) duration and heightened slow-wave activity during non-REM (NREM) sleep. Circadian misalignment is common, with children showing evening chronotypes and difficulty initiating sleep. These disturbances exacerbate core ADHD symptoms—particularly inattention and impulsivity—create a vicious cycle of sleep deprivation and daytime dysfunction, and adversely affect academic performance, emotional regulation and family wellbeing. Emerging evidence implicates multifactorial aetiologies: dysregulated dopaminergic and frontostriatal circuitry, genetic susceptibility linked to neurotransmission and circadian genes, and environmental factors such as inconsistent bedtime routines. Interventions range from behavioural sleep hygiene and tailored cognitive–behavioural strategies to optimised pharmacotherapy and exploratory neurofeedback approaches, each seeking to restore sleep continuity and improve daytime functioning.
