Summary

Shift work, encompassing night shifts, rotating schedules and extended hours, is integral to modern healthcare, transportation, manufacturing and emergency services. Disruption of endogenous circadian rhythms and sleep–wake cycles leads to fragmented sleep, reduced total sleep time and altered sleep architecture. Chronic exposure to such disturbances is linked to heightened risks of metabolic syndrome, cardiovascular disease, impaired immune function and mood disorders. Equally consequential are deficits in alertness and cognitive performance, which compromise safety, decision-making and overall productivity. Recent efforts focus on optimising shift schedules, incorporating strategic napping and light exposure, and tailoring interventions to individual vulnerabilities. An improved understanding of physiological markers and behavioural moderators is critical for the development of evidence-based countermeasures that safeguard the health of shift workers and maintain operational efficacy across industries.

Foundational work has quantified the extent of sleep restriction and circadian misalignment in rotating shift workers, demonstrating that consecutive night and early morning shifts yield significant reductions in sleep duration compared with day shifts. Objective assessment using wrist actigraphy and urinary melatonin markers revealed that performance impairments on vigilance tasks peak during the first night shift and persist on subsequent nights, correlating with the timing of the circadian acrophase. These findings underscore the absence of full circadian adaptation to night work and highlight the need for shift designs that minimise rapid transitions between shift types to preserve cognitive function and safety.
