Summary
The human central nervous system is bathed in cerebrospinal fluid (CSF), which is produced by the choroid plexus, circulates through ventricular and subarachnoid spaces, and is reabsorbed via arachnoid granulations. This tightly regulated circulation maintains intracranial pressure within narrow limits, ensuring tissue perfusion and metabolic clearance. Disturbances in CSF volume or flow—whether by dural tears, venous fistulae or impaired absorption—can lead to intracranial hypotension or hypertension. Hypotension often presents as orthostatic headache, brain sagging and meningeal enhancement, whereas hypertension may manifest with headache, visual disturbance and cognitive impairment. Advances in non-invasive and invasive imaging, coupled with refined physiological monitoring, have deepened our understanding of pressure–volume dynamics and the compensatory roles of venous and lymphatic pathways. Therapeutic strategies now range from epidural blood patching and targeted endovascular embolisation to shunting procedures tailored to restore homeostasis. A multidisciplinary focus on early diagnosis, individualised intervention and long-term outcome assessment is critical for reducing morbidity and improving quality of life in affected patients worldwide.

Decubitus CT myelography: A CT myelogram performed in a lateral position to exploit gravity for the enhanced detection of subtle CSF leaks.
