Spontaneous Regression Phenomena in Hepatocellular Carcinoma
Summary
Spontaneous regression of hepatocellular carcinoma (HCC) represents a rare clinical event in which established tumours partially or completely disappear in the absence of specific anticancer therapy. Observed in fewer than 1% of all HCC cases, these regressions have been documented across a wide age range and diverse aetiologies, including viral hepatitis, alcohol-related liver disease and non-alcoholic steatohepatitis. Macroscopic and histological examinations consistently reveal extensive coagulative necrosis, fibrous encapsulation and obliteration of feeding vessels, suggesting that tumour autoinfarction, ischaemia and immune-mediated cytotoxicity may act alone or in concert. Proposed mechanisms include tumour hypoxia secondary to vascular thrombosis, enhanced host inflammatory responses with activation of cytotoxic lymphocytes and cytokine release, and metabolic stress induced by underlying comorbidities such as renal failure. Although individual case reports and small series dominate the literature, emerging examples highlight the potential to harness these natural regression pathways for therapeutic innovation. Clinically, spontaneous regression may influence surgical decision-making, refine transplant candidacy and prompt investigation of predictive biomarkers. A deeper understanding of this phenomenon could reveal new avenues for immunomodulation, targeted vascular interventions and personalised management of advanced HCC.
