Summary
Statins, a class of 3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitors widely prescribed to lower cholesterol, have attracted considerable interest for their potential influence on skeletal integrity. Preclinical studies reveal that statins may promote osteoblast differentiation and inhibit osteoclast activity, suggesting an anabolic effect on bone formation. Observational and interventional clinical studies, however, present a complex picture: some investigations report modest enhancements in bone mineral density and reductions in fracture incidence, while others find negligible or even adverse outcomes in specific populations. Dose, statin lipophilicity and patient characteristics such as age and comorbidities emerge as important modulators of skeletal response. Mechanistic insights implicate pathways including bone morphogenetic protein signalling and modulation of bone turnover markers. Taken together, current evidence points to a nuanced role for statins in bone health, with potential benefits tempered by variability across study designs and patient cohorts.
