Summary

Subacute sclerosing panencephalitis (SSPE) is a rare, progressive neurodegenerative disorder resulting from a persistent, aberrant infection of the central nervous system by the measles virus. It typically manifests several years after acute measles infection and is characterised by cognitive decline, behavioural changes, myoclonic jerks and eventual progression to a vegetative state. The disease arises when mutated viral particles accumulate within neurons and glial cells, triggering chronic inflammation, neuronal loss and demyelination. Although widespread measles immunisation has markedly reduced SSPE incidence, outbreaks driven by vaccine hesitancy and gaps in coverage continue to pose a global threat. Therapeutic options remain largely supportive—combining antiviral agents, immunomodulatory treatments and symptomatic relief—with no definitive cure currently available. Ongoing research focuses on unraveling viral persistence mechanisms, host immune responses and novel intervention strategies to prevent and manage this invariably fatal condition.

Subacute sclerosing panencephalitis (SSPE): A fatal neurological disorder caused by persistent measles virus infection in the central nervous system.

Myoclonus: Sudden, involuntary muscle jerks characteristic of SSPE’s clinical presentation.

Persistent infection: A chronic state in which a virus evades host clearance and remains within tissues over an extended period.

Low glycaemic index therapy: A dietary strategy that limits rapid glucose fluctuations, investigated as an adjunctive treatment to ameliorate SSPE symptoms.
