Summary
Osteoarthritis is increasingly recognised as a disease of the entire joint organ, with subchondral bone remodelling playing a central role in both symptom generation and disease progression. Early in the disease process, increased bone turnover and microstructural changes in the subchondral plate lead to bone marrow lesions, altered mechanical loading and crosstalk with overlying cartilage. Progressive sclerosis of the subchondral plate, osteophyte formation and vascular invasion further destabilise the osteochondral unit, perpetuating cartilage degradation. Targeting subchondral bone remodelling therefore offers a promising avenue for disease modification, with pharmacological approaches ranging from antiresorptive bisphosphonates and selective oestrogen receptor modulators to anabolic agents and combination therapies. Advances in imaging biomarkers and molecular profiling are refining patient stratification, enabling early intervention in those most likely to benefit. By stabilising subchondral bone architecture and modulating inflammatory mediators, tailored treatments hold potential to delay joint replacement and improve quality of life on a global scale.

In a preclinical model of post-traumatic osteoarthritis induced by medial meniscus destabilisation in rats, a novel hyaluronic acid–alendronate conjugate was administered via a single intra-articular injection four weeks after injury. This treatment significantly attenuated cartilage degeneration, lowered expression of matrix metalloproteinases and pro-inflammatory cytokines in subchondral bone, and improved pain thresholds compared with controls. Serum markers of bone resorption were also reduced. These findings demonstrate that local delivery of bisphosphonate in a hyaluronan carrier can limit osteochondral damage and support the development of targeted intra-articular strategies for osteoarthritis management.
