Summary
Subchorionic haematomas are localised collections of blood between the chorionic membrane and the uterine wall, detected by ultrasonography in early gestation. They arise from partial detachment of the chorion and are observed in up to 3–5% of pregnancies. While many resolve without consequence, their presence has been linked to elevated risks of first‐trimester miscarriage, preterm labour, fetal growth restriction and placental disorders. The size, location and duration of haematomas appear to modulate risk, with larger or persistent lesions correlating with adverse outcomes. The underlying mechanisms involve disruption of placental implantation and local inflammatory responses, which may predispose to hypertensive disorders and abnormal placental adhesion. Management strategies centre on heightened surveillance, including serial scans and tailored obstetric care, although uniform treatment guidelines remain under development.
