Substance Use Disorders and Treatment Interventions

Substance use disorders (SUDs) encompass a range of chronic relapsing conditions characterised by compulsive drug seeking and consumption despite harmful consequences. They arise from complex interactions between genetic predisposition, neurobiological adaptations—particularly within mesolimbic dopaminergic circuits—and environmental stressors. Globally, the burden of alcohol, opioid, stimulant and cannabis use disorders continues to rise, contributing substantially to morbidity, mortality and social harm. Standard treatment paradigms combine psychosocial therapies, such as cognitive behavioural approaches and motivational interviewing, with pharmacological interventions including methadone, buprenorphine and long-acting naltrexone. Harm reduction strategies—needle-and-syringe programmes, supervised consumption settings and overdose education with naloxone distribution—have demonstrated effectiveness in mitigating acute risks. Recent advances seek to refine treatments by integrating precision-medicine tools, digital health platforms for real-time monitoring and novel neuromodulation techniques, all aimed at improving long-term recovery and reducing relapse.

Recent studies employing single-cell transcriptomics have delineated cell-type-specific adaptations within the ventral tegmental area and nucleus accumbens following chronic opioid and stimulant exposure, illuminating molecular candidates for targeted blockade of pathologic synaptic plasticity. A multicentre trial of a biodegradable subcutaneous implant delivering buprenorphine over six months reported superior retention in care and lower rates of illicit opioid use compared with daily oral formulations. In parallel, pilot work on closed-loop neurofeedback using functional magnetic resonance imaging has demonstrated the feasibility of reinforcing self-regulated activity in prefrontal circuits, reducing cue-induced craving in individuals with alcohol use disorder while offering a non-pharmacological adjunct to standard therapy.
