Summary

Intermittent exotropia is a form of outward ocular misalignment characterised by episodic divergence of one eye, principally affecting children and often persisting into adulthood. Surgical strategies aim to restore ocular alignment by altering extraocular muscle tension and positioning, thereby improving binocular function and depth perception. Standard techniques include recession of the lateral rectus muscle and resection of the medial rectus muscle, either singly or in combination, tailored to the magnitude of deviation. Patient selection relies on assessment of deviation angle, control scores, sensory status and fusion reserves. Longitudinal outcomes reveal high rates of initial motor success, although sensory success, defined by restoration of stereopsis, remains variable. Refinements in surgical dosing and tailored approaches for moderate versus large-angle deviations have enhanced long-term alignment stability and reduced reoperation rates, underscoring the importance of individualised surgical planning.

Another seminal report has charted the natural course of intermittent exotropia in a cohort of children followed over three years, identifying fusional convergence parameters as predictors of control deterioration. This work highlights the potential role of non-surgical factors in guiding timing and choice of surgery to optimise long-term alignment and sensory outcomes.

Technical terms

Intermittent exotropia: A form of strabismus characterised by episodic outward deviation of one eye, with periods of normal alignment.

Prism diopter (PD): A unit of measurement for ocular deviation, where 1 PD deflects light by 1 cm at a distance of 1 m.

Lateral rectus recession: A surgical procedure that weakens the lateral rectus muscle by repositioning its attachment further back on the eye.

Recess–resect procedure: A combined surgery involving weakening one extraocular muscle (recession) and strengthening its antagonist (resection) to correct ocular misalignment.

Stereoacuity: The smallest detectable depth difference perceived by binocular vision, reflecting the quality of binocular sensory function.
