Summary

Surgical management of rib fractures and thoracic trauma has evolved from purely conservative measures towards targeted operative interventions aimed at restoring chest wall stability, reducing pain and accelerating respiratory recovery. Key developments include the refinement of indications for open reduction and internal fixation of displaced or flail rib segments, integration of multimodal analgesia pathways and adoption of multidisciplinary clinical protocols. Operative stabilisation is now recognised to shorten mechanical ventilation times, reduce pulmonary complications and improve long‐term quality of life in selected patients. Trends in patient selection emphasise the number and displacement of fractures, underlying pulmonary contusion and comorbidities, while advances in imaging and operative techniques have broadened the scope of surgical candidacy. Technical terms

Flail chest: A segment of the thoracic cage where three or more consecutive ribs are fractured in two or more places, causing paradoxical movement during respiration.

Open reduction and internal fixation (ORIF): Surgical realignment of fractured ribs followed by stabilisation with plates or other hardware to restore chest wall integrity.

Pulmonary contusion: Injury to the lung parenchyma resulting in alveolar haemorrhage and oedema, commonly accompanying rib fractures and impairing gas exchange.

Pneumothorax: Accumulation of air in the pleural space, which can collapse the lung and often necessitates chest tube drainage.

Thoracic Trauma Severity Score (TTSS): A composite index used to assess the severity of chest wall injury, incorporating factors such as number of fractures, displacement and associated pulmonary findings.
