Summary

Ulnar nerve entrapment at the elbow represents the second most common compressive neuropathy of the upper limb. Presentation ranges from intermittent paraesthesia in the ulnar digits to progressive sensory loss and intrinsic hand muscle weakness. Initial management is conservative, encompassing activity modification, splintage and physiotherapy. Surgery is indicated for refractory or severe cases and aims to relieve compressive forces while preserving vascular supply. The principal techniques include simple in situ decompression, anterior transposition (subcutaneous, intramuscular or submuscular) and medial epicondylectomy. Selection depends on patient factors, nerve stability and anatomical considerations. Endoscopic and minimal-incision approaches have gained traction for reduced morbidity. Key challenges remain optimal patient selection, predicting functional recovery and minimising complications such as neuropathic pain or iatrogenic injury. Recent efforts focus on refining perioperative protocols, integrating advanced imaging for surgical planning and tailoring interventions to comorbidities such as diabetes. Longitudinal outcome measures now incorporate both objective electrophysiological parameters and patient-reported instruments, providing a granular understanding of recovery trajectories and informing shared decision-making.

Another longitudinal cohort investigation characterised postoperative recovery following anterior subcutaneous transposition. Patient-reported arm, shoulder and hand disability scores improved significantly by six months, while sensory and motor conduction velocities demonstrated measurable gains within three months. These data delineate expected timelines for symptom resolution and strength restoration, offering clinicians benchmarks for postoperative counselling.
