Summary
Syndecan-1 is a transmembrane heparan sulphate proteoglycan widely expressed on epithelial and endothelial cells. Through its extracellular chains it binds growth factors, chemokines, extracellular matrix components and microbial virulence factors, thereby orchestrating cell–cell and cell–matrix interactions. Dynamic regulation of syndecan-1 occurs via ectodomain shedding, a proteolytic process that releases its soluble form to act at distant sites. On the cell surface, intact syndecan-1 supports barrier integrity, modulates receptor signalling and dampens excessive inflammatory cascades. Once shed, the ectodomain can sequester cytokines or promote ligand presentation, influencing neutrophil recruitment, fibroblast activation and tissue remodelling. Dysregulation of this balance contributes to chronic inflammatory conditions, fibrotic disorders and tumour progression. Increasingly, both membrane-anchored and soluble syndecan-1 are recognised as biomarkers of disease severity and as potential therapeutic targets to tune inflammatory responses and promote tissue repair.
