Summary
T cells are central to the pathogenesis of various forms of liver injury. Upon antigen presentation by hepatocytes or non-parenchymal antigen-presenting cells such as Kupffer cells and liver sinusoidal endothelial cells, naive T cells differentiate into effector subsets. CD8+ cytotoxic T lymphocytes mediate hepatocyte damage via perforin/granzyme release and FAS ligand interactions, whereas CD4+ Th1 cells secrete pro-inflammatory cytokines such as interferon-γ and tumour necrosis factor-α. Chronic activation of T cells also drives oxidative stress, apoptotic signalling and stellate cell activation, thereby promoting fibrogenesis. Interactions between T cells and resident hepatic cells create a microenvironment that modulates tolerance and inflammation in response to viral infection, autoimmune triggers or toxic insults. Dysregulation of regulatory T cells contributes to the breakdown of self-tolerance in autoimmune hepatitis. Understanding the molecular mechanisms governing T cell activation, trafficking and effector functions is critical for developing targeted immunomodulatory therapies with broad applications from viral hepatitis to drug-induced liver injury.
