Summary
Temporal lobe epilepsy (TLE) represents the most prevalent focal epilepsy in adults, characterised by recurrent seizures arising from medial temporal structures, especially the hippocampus. Pathogenic mechanisms include an imbalance between excitatory glutamatergic and inhibitory GABAergic transmission, hippocampal sclerosis with neuronal loss and reactive gliosis, synaptic reorganisation and neuroinflammatory cascades that perpetuate network hyperexcitability. Ionic channel dysfunction and excessive intracellular calcium play central roles in excitotoxic injury, while alterations in blood–brain barrier integrity and metabolic pathways further promote epileptogenesis. Current management relies on antiseizure medications targeting ion channels and neurotransmitter receptors, dietary therapies such as ketogenic diets and surgical resection in pharmacoresistant cases. Emerging strategies seek to modulate neuroinflammation, enhance neuromodulation through electrical or molecular interventions, identify fluid biomarkers for early diagnosis and apply gene or cell therapies to restore network homeostasis. Together, these insights pave the way for personalised approaches that address both seizure control and the underlying disease mechanisms.
