Testicular Germ Cell Tumor Management and Treatment
Summary
Testicular germ cell tumours represent the most common malignancy in young men and are broadly classified into seminomas and non-seminomas. Primary management involves radical orchiectomy for histological diagnosis followed by accurate staging to guide risk-adapted therapy. Surveillance strategies address stage I disease with regular imaging and tumour marker assessments, while adjuvant chemotherapy or radiotherapy is reserved for higher-risk presentations. Metastatic disease is highly curable owing to remarkable sensitivity to cisplatin-based combination regimens, though 10–20% of patients exhibit refractory or relapsed disease requiring intensification or salvage approaches. Long-term survivorship considerations include fertility preservation, management of late toxicities such as cardiovascular and secondary malignancies, and integration of novel biomarkers to refine prognostication. Advances in molecular stratification, liquid biopsy, immunotherapeutic targets and precision medicine are converging to individualise treatment while minimising morbidity.

Recent single-cell multi-omics profiling of seminoma tissues has generated a spatial atlas of tumour cell programmes and the immune landscape, revealing shared transcriptional circuits between malignant germ cells and their embryonic precursors. Key regulators such as TFAP2C have been implicated in tumour invasiveness, while distinct immune subpopulations with exhaustion phenotypes localise proximal to malignant regions, suggesting avenues for microenvironment-directed therapies. Complementing this, whole-exome sequencing studies have characterised the low mutational burden of germ cell tumours, confirmed recurrent alterations including 12p amplification and KIT mutations, and identified novel drivers such as CDC27 and FSIP2. Importantly, mutations in DNA-repair genes like XRCC2 have been linked to cisplatin resistance, offering molecular entry points for overcoming therapeutic failure.

Tumour microenvironment (TME): The complex milieu of immune cells, stromal elements and signalling molecules surrounding cancer cells, which influences progression and therapeutic response.
