Testicular Torsion Diagnosis and Management Strategies
Summary
Testicular torsion results from twisting of the spermatic cord, leading to venous outflow obstruction, arterial ischaemia and potential testicular loss. It typically manifests as sudden, severe scrotal pain, often accompanied by nausea, vomiting and an elevated, transversely oriented testis. Absent or diminished cremasteric reflex and a high-riding testis raise clinical suspicion. Colour Doppler ultrasound is the principal imaging modality for assessing testicular perfusion, yet false-negative results may occur in cases of partial torsion or early presentation. Surgical exploration remains the definitive diagnostic and therapeutic step: detorsion followed by bilateral orchidopexy for viable testes and orchiectomy when viability is irreversibly compromised. Manual detorsion can serve as an interim measure when operative access is delayed. Prognostic determinants include symptom duration, degree of cord rotation and rapidity of specialist referral. Recent efforts focus on optimising transfer pathways, reinforcing ultrasound protocols and expanding diagnostic training in non-specialist settings. Underpinning these strategies is the imperative that “time is testis,” with global initiatives exploring telemedicine support and triage algorithms to improve salvage rates and reduce long-term sequelae.

A large cross-sectional study of over one thousand patients undergoing scrotal exploration identified critical factors influencing delay and salvage. Misdiagnosis by junior and non-urology practitioners, consultation delay with non-specialists and absence of detectable blood flow on Doppler imaging were all associated with surgery beyond the optimal six-hour window. An absent cremasteric reflex paradoxically marked earlier intervention, while a cord twist exceeding 180° and febrile presentation predicted testicular loss. This research underscores the value of extending diagnostic training and simulation exercises to primary and secondary care clinicians to enhance early recognition and referral.
