Summary
Theory of Mind (ToM) refers to the capacity to attribute mental states—beliefs, desires, intentions and emotions—to oneself and to others, and to understand that these mental states may differ from one’s own. In autism spectrum disorders (ASD), a core characteristic is the atypical development of ToM, which can manifest as challenges in social communication, perspective taking and adaptive interaction. Research has demonstrated that individuals with ASD often show delays or differences in false-belief understanding, pragmatic language use and spontaneous mentalising, even when they perform within typical ranges on explicit tasks. Neuroimaging studies have implicated regions such as the right temporoparietal junction and anterior temporal poles in mediating these functions, with variations in activation patterns seen in autistic compared to neurotypical individuals. Heterogeneity in ToM skills is a hallmark of ASD across the lifespan, influenced by factors including verbal ability, cognitive style and social motivation. Advances in assessment have sought to capture both explicit and implicit components of mentalising, and to develop scalable, objective tests for diverse age groups. Understanding the nuances of ToM in ASD has global significance for educational strategies, clinical diagnosis and the design of interventions that support social learning and communication in real-world contexts.
