Therapeutic Strategies in Kidney Disease Management

Summary

Kidney disease encompasses acute and chronic conditions that compromise renal filtering capacity, disrupt fluid and electrolyte balance and increase cardiovascular risk. Traditional management has concentrated on blockade of the renin-angiotensin-aldosterone system, glycaemic control in diabetes and blood pressure optimisation. In recent years, the repertoire has expanded to include sodium-glucose cotransporter-2 inhibitors, endothelin receptor antagonists and non-steroidal mineralocorticoid receptor antagonists. At the same time, targeted approaches such as anti-fibrotic agents, anti-inflammatory drugs and modulators of oxidative stress have been pursued to halt progression of renal fibrosis and preserve nephron integrity. Emerging strategies range from nanomedicine-based drug delivery to precision modulation of cellular pathways such as autophagy and inflammasome activation. Biological therapies, including cell-based repair and RNA interference, aim to repair injured tubules or podocytes. The gut–kidney axis has also become a focus, with microbiota-directed treatments to mitigate uraemic toxicity. Taken together, current research seeks to marry established systemic therapies with novel interventions that address the molecular underpinnings of kidney injury, offering personalised and organ-protective regimens.

Recent studies have elucidated molecular mechanisms by which bioactive plant extracts and their constituents alter key intracellular pathways in chronic kidney disease. One seminal investigation demonstrated that a naturally derived anthraquinone compound inhibits excessive autophagy and reduces renal fibrosis by modulating AMPK-dependent mTOR signalling in tubular cells. This work provided proof of principle that targeting autophagy can attenuate fibrotic remodelling and preserve tubular architecture, thereby opening avenues for small-molecule interventions in established disease.

Technical terms

Chronic kidney disease (CKD): A long-term decline in renal function characterized by reduced glomerular filtration rate or markers of kidney damage lasting over three months.

Acute kidney injury (AKI): A rapid loss of kidney function over hours to days, often reversible but associated with high morbidity if unaddressed.

Podocyte: A specialised epithelial cell in the glomerulus critical for maintaining the filtration barrier and preventing proteinuria.

Autophagy: A regulated cellular process that degrades and recycles damaged organelles and proteins, which can be protective or maladaptive in kidney cells.

Renal fibrosis: Accumulation of extracellular matrix in the interstitium and glomeruli that leads to scarring and irreversible loss of nephron units.
