Thrombophilia Management in Adverse Pregnancy Outcomes

Summary

Thrombophilia is a predisposition to excessive blood clotting that can compromise placental perfusion and lead to adverse pregnancy outcomes including recurrent miscarriage, pre-eclampsia, placental abruption and intrauterine growth restriction. Management strategies traditionally focus on risk stratification based on inherited or acquired thrombophilic factors, followed by tailored anticoagulant prophylaxis. Low molecular weight heparin (LMWH) and low-dose aspirin are the mainstays of therapy, aiming to restore haemostatic balance, improve trophoblast invasion and maintain adequate uteroplacental blood flow. Clinical decision-making incorporates maternal history, genetic testing and serological profiles to identify women at greatest risk. Recent advances have refined diagnostic criteria for both major and minor thrombophilias, introduced algorithms for personalised therapy and clarified the immunomodulatory effects of anticoagulants. Despite progress, heterogeneity in study designs and variable quality of evidence continue to challenge consensus on optimal regimens, dosing and timing. A global perspective emphasises the need for harmonised guidelines that address diverse patient populations and resource settings. Future directions include biomarker-driven approaches and integrated maternal-foetal monitoring to further mitigate risks and enhance live birth rates.

Recent systematic reviews have re-evaluated the effectiveness of LMWH in women with unexplained recurrent pregnancy loss. Meta-analyses of randomised trials indicate that routine LMWH administration, with or without concurrent low-dose aspirin, may not significantly improve live birth rates in the absence of clear thrombophilic markers, underscoring the need for more rigorous stratification of patient subgroups. In parallel, mechanistic in vitro studies have examined the immunological actions of LMWH on central regulatory cells. Exposure to LMWH altered macrophage polarisation and T helper cell profiles, promoting a pro-inflammatory milieu that could facilitate early implantation processes. These findings suggest that beyond anticoagulation, LMWH may exert gestational stage-dependent immunomodulatory effects, offering insight into its potential benefits and guiding more nuanced therapeutic windows.
