Summary

Tissue expansion in bone augmentation encompasses strategies to generate surplus soft tissue prior to grafting or implant placement, thereby ensuring tension-free closure and minimising postoperative complications. Traditional mechanical expanders employ silicone balloons inserted beneath the periosteum and progressively inflated to induce adjacent tissue growth. More recently, self-inflating osmotic hydrogel expanders have emerged, exploiting polymer swelling in situ to achieve controlled volume increase without external adjustments. These systems are typically implanted via a minimally invasive subperiosteal tunnelling technique and allowed to expand over days to weeks, creating a soft tissue envelope suited to subsequent bone graft or guided bone regeneration. Advances in polymer chemistry have enabled hydrogels with tailored induction periods, linear swelling profiles and biocompatible degradation, enhancing predictability and patient comfort. Clinically, tissue expansion has been applied to vertical and horizontal alveolar ridge reconstruction, management of severely atrophic edentulous ridges and preparation for endosseous implants in both maxillary and mandibular sites. Integration with autologous bone, xenograft or synthetic substitutes underpins current protocols, with emerging injectable hydrogels offering further potential for ambulatory application and reduced surgical morbidity.

Guided bone regeneration (GBR): A technique using barrier membranes to direct the growth of new bone tissue into a defined defect site, often following tissue expansion.

Alveolar ridge: The bony crest of the jaw that supports teeth and dental implants, often requiring augmentation in cases of resorption or atrophy.
