Summary

Tobacco consumption in both smoked and smokeless forms exerts a multifaceted impact on the oral cavity, manifesting in elevated risk of dental caries, periodontal disease, mucosal lesions and impaired wound healing. The introduction of carcinogenic compounds and reactive free radicals disrupts local immune responses and diminishes salivary flow and buffering capacity. Nicotine and other substances promote vascular constriction, reducing tissue perfusion and delaying repair, while chemical irritation fosters leukoplakia, erythroplakia and malignancies of the lip, tongue and oral mucosa. Alterations in the oral microbiome further encourage overgrowth of acidogenic and proteolytic organisms, exacerbating tooth decay and gum inflammation. These effects are observed globally, with variations in prevalence according to product type, regional patterns of use and socioeconomic context. Public health measures focused on cessation, early detection and microbiome restoration are essential to mitigate this preventable burden.

Recent studies have applied quantitative synthesis methods to assess the health impacts of chewing tobacco, revealing a moderate association with lip and oral cavity cancer, pharyngeal malignancies and stroke, though evidence for cardiovascular outcomes such as ischaemic heart disease remains inconclusive. Conservative risk estimates underscore the need for strengthened policy interventions and targeted research to refine exposure–response relationships. In parallel, high-resolution sequencing of the supragingival microbiome in cigarette and alternative tobacco smokers has demonstrated type-specific dysbiosis, with increased abundance of Streptococcus mutans and Lactobacilli in heavy smokers and enrichment of periodontopathogens in users of non-conventional products. These findings implicate shifts in microbial community structure as a key driver of caries susceptibility and suggest avenues for microbiota-based preventive therapies.
