Tocilizumab Treatment Strategies for Severe COVID-19
Summary
Severe COVID-19 is characterised by dysregulated immune activation, often culminating in a hyperinflammatory syndrome driven in large part by interleukin-6 (IL-6). Tocilizumab, a humanised monoclonal antibody targeting the IL-6 receptor, has been investigated as a means to attenuate this maladaptive response, with the aim of reducing respiratory failure, organ dysfunction and mortality. Clinical interest initially stemmed from observations that pathogenic T cells and inflammatory monocytes produced elevated IL-6 levels, contributing to vascular leakage and acute respiratory distress. By blocking IL-6 signalling, tocilizumab may mitigate the “cytokine storm”, facilitate restoration of lymphocyte counts, lower acute-phase reactants and hasten resolution of pulmonary infiltrates. Treatment strategies have varied in timing, dosing (single versus repeated infusions of 400–800 mg) and patient selection criteria, notably baseline hypoxia and markers of systemic inflammation such as C-reactive protein. Tocilizumab has been administered alongside standard-of-care therapies, including corticosteroids, anticoagulation and antiviral agents. Evidence to date supports a reduction in progression to mechanical ventilation and improved survival in selected hospitalised patients, while recognising that earlier intervention, appropriate patient stratification and vigilant monitoring for secondary infections are critical to maximising benefit and minimising risk.
