Tolerance of Uncertainty in Medical Education
Summary
Tolerance of uncertainty in medical education refers to the capacity of learners and practitioners to recognise, engage with and manage situations in which information is incomplete, ambiguous or evolving. This faculty underpins clinical reasoning, diagnostic decision-making and professional adaptability. Low tolerance of uncertainty has been associated with higher levels of stress, burnout and defensive practice, whereas higher tolerance correlates with resilience, reflective learning and patient-centred care. Medical curricula have begun to address this construct through explicit instruction in risk assessment, reflective practice and communication strategies, as well as through experiential modalities such as simulation and interprofessional learning. By fostering an environment in which uncertainty is acknowledged rather than concealed, educators aim to prepare students and trainees to navigate the inherent unpredictability of clinical practice, to mitigate over-investigation and to strengthen patient trust. Globally, the development of tolerance of uncertainty has emerged as a key competency for safe, adaptive and compassionate healthcare.

Tolerance of Uncertainty in Medical Education publication trend
The graph below shows the total number of articles in tolerance of uncertainty in medical education across all publications each year (not limited to Nature Index journals).
Technical terms
Tolerance of uncertainty: The ability to recognise and remain effective in situations where outcomes are unpredictable and data are incomplete.
Tolerance of ambiguity: A related construct denoting comfort with information that is vague, conflicting or open to multiple interpretations.
Epistemic fidelity: A characteristic of simulation design that preserves the authentic complexity and uncertainty of clinical practice.
Simulation-based learning: An educational approach using realistic scenarios and role-play to develop clinical and decision-making skills in a safe environment.
Reflective practice: A deliberate process of self-examination and critical analysis of one’s experiences to inform future practice and learning.
