Trade Policy and Public Health Implications

Trade policy shapes the movement of goods, services and capital across borders, with direct consequences for population health. International agreements and domestic regulations determine the availability, affordability and nutritional quality of food, the marketing of health-harmful commodities and access to medicines. Liberalisation of tariffs and non-tariff barriers can enhance dietary diversity and reduce malnutrition in some contexts, yet may also facilitate the proliferation of processed foods, tobacco and alcohol, elevating the burden of non-communicable diseases. Foreign direct investment alters local production and distribution networks, influencing consumer environments and risk exposures. At the same time, legal provisions in trade and investment treaties—such as intellectual property rights and investor–state dispute settlement mechanisms—can constrain public health policy space and lead to regulatory chill. A nuanced understanding of how specific trade modalities and implementation challenges interact with health outcomes is essential for designing health-sensitive trade agreements and safeguarding the right to regulate for public welfare.

Recent studies have demonstrated that international food trade exerts a significant influence on global dietary risks and mortality. Analyses for 2019 indicate that imports of fruits, vegetables, legumes and nuts contributed to an estimated reduction of 1.4 million deaths from non-communicable diseases worldwide, whereas red meat imports were associated with approximately 150,000 additional deaths. These findings underscore the importance of incorporating health impact assessments into trade and agricultural policies, revealing that the net public health impact of food trade depends critically on the nutritional profile of traded commodities.
