Summary
Transcranial Doppler ultrasonography (TCD) is a non-invasive technique that employs pulsed ultrasound to measure blood flow velocity within the basal cerebral arteries. In the context of acute ischaemic stroke, TCD offers real-time assessment of cerebral haemodynamics, enabling rapid identification of arterial occlusions, monitoring of collateral circulation, and evaluation of reperfusion during thrombolysis or endovascular intervention. Its portability and safety profile make it well suited to both in-hospital and prehospital environments, where early detection of large vessel occlusion (LVO) can guide triage to comprehensive stroke centres. Beyond occlusion detection, TCD can track dynamic changes in flow during clot retrieval, predict reperfusion injury, and signal the onset of vasospasm or microemboli. Recent advances in signal processing, deep learning and automated beat quality assessment have enhanced the reliability and operator independence of TCD measurements. Despite limitations related to acoustic window penetration and operator training, ongoing technical innovations and predictive modelling promise to broaden clinical uptake, optimise stroke pathways and improve patient outcomes on a global scale.
