Transcranial Magnetic Stimulation Applications in Neuropathic Pain Management

Summary

Neuropathic pain arises from lesions or dysfunction in the somatosensory system and often proves refractory to conventional pharmacotherapy. Transcranial magnetic stimulation (TMS) offers a non-invasive means to modulate cortical excitability through induction of electric currents in targeted brain regions. Repetitive TMS (rTMS), delivered at varying frequencies, can engage descending inhibitory pathways, alter neurotransmitter release and reduce central sensitisation. Clinical protocols typically focus on the primary motor cortex (M1) or dorsolateral prefrontal cortex, with high-frequency stimulation over M1 shown to yield the most consistent analgesic benefit. Maintenance regimens extend initial induction sessions with periodic booster treatments to sustain relief. Despite promising safety and tolerability profiles, heterogeneity in stimulation parameters and in patient populations has complicated the establishment of standardised clinical guidelines. Ongoing studies aim to refine coil designs, targeting strategies and biomarkers of response to optimise individual outcomes and integrate rTMS into multidisciplinary pain management pathways worldwide.

Recent studies have evaluated navigation-guided high-frequency rTMS over M1 in patients with chronic neuropathic pain of the upper limb. In a sham-controlled, parallel-group trial, daily 10 Hz stimulation for one week followed by weekly maintenance did not achieve a statistically significant separation in mean pain diary scores at week 9, but trends favoured active rTMS in reducing pain-related disability. Safety was confirmed by the absence of serious adverse events, and improvements in functional measures suggest that rTMS may enhance quality of life even when analgesic thresholds are not uniformly met. These findings underscore the potential of personalised navigation and optimised dosing schedules to improve clinical efficacy in focal neuropathic pain syndromes.
