Transient Global Amnesia and Cognitive Impairment

Summary

Transient global amnesia is a clinical syndrome characterised by the abrupt onset of profound anterograde amnesia, typically lasting less than 24 hours, in otherwise healthy middle-aged or older adults. During an episode, patients are unable to form new memories yet retain personal identity and procedural skills, often accompanied by mild anxiety or disorientation. Although the amnestic period resolves completely, subtle cognitive sequelae have been documented, including accelerated long-term forgetting and impairments in autobiographical recollection. The aetiology remains uncertain, with proposed mechanisms spanning transient ischaemia, venous outflow obstruction and network dysfunction within the limbic system. Differential diagnoses include transient epileptic amnesia, cerebrovascular events and migraine-associated phenomena. While standard neuroimaging is often unremarkable, diffusion-weighted imaging may reveal hippocampal lesions in a minority. Most patients experience a single event, but around 10–20 % may have recurrences, with factors such as head injury, mood disorders and family history of dementia implicated. Despite the alarming presentation, long-term global cognitive outcomes are generally favourable, though the risk of subsequent memory complaints or mild cognitive changes warrants ongoing surveillance.

Recent investigations have probed the boundary between normal ageing and pathological memory decline by assessing long-term forgetting rates across the lifespan. One study in healthy adults spanning 18 to 77 years measured verbal episodic retention over one week and found that, despite age-related reductions in initial learning and sleep efficiency, older participants did not exhibit accelerated forgetting compared to younger counterparts. These findings suggest that disproportionate long-term forgetting is not an inevitable feature of healthy ageing but may serve as an early marker for underlying neuropathology. Such work refines our understanding of memory consolidation and emphasises the importance of distinguishing normal cognitive ageing from the memory disturbances observed in transient global amnesia and related conditions.

Anterograde amnesia: The inability to form new episodic memories following the onset of the syndrome.

Accelerated long-term forgetting (ALF): A condition in which information initially learned and retained over standard intervals is lost at an abnormally rapid rate over subsequent hours or days.

Limbic circuit: A network of interconnected brain regions, including the hippocampus, amygdala, cingulate gyrus and orbitofrontal cortex, critical for memory formation and emotional regulation.

Memory consolidation: The neurobiological process by which initially labile memory traces are stabilised and integrated into long-term storage.
