Summary

Trauma resuscitation in the context of haemorrhagic shock encompasses a continuum of interventions aimed at arresting bleeding, restoring circulatory volume and preserving organ function. Initial management prioritises rapid haemorrhage control through surgical, interventional or mechanical means while maintaining perfusion with judicious fluid and blood‐product administration. A growing emphasis on damage‐control resuscitation has shifted practice away from large‐volume crystalloid infusion towards balanced ratios of red blood cells, plasma and platelets to mitigate dilutional coagulopathy and endotheliopathy. The adoption of predefined transfusion protocols and real‐time physiological targets, such as mean arterial pressure and lactate clearance, guides tailored therapies. Advances in prehospital care, point‐of‐care coagulation testing and computational modelling now facilitate individualised strategies. Collectively, these developments carry global significance by reducing preventable mortality, standardising practice across diverse settings and informing resource allocation in both civilian and military trauma systems.

Innovative computational frameworks have emerged that bridge laboratory and clinical realms via a three‐compartment ordinary differential equation model of inflammation and coagulation. By integrating porcine haemorrhagic shock data with observations from a multicentre trauma cohort, the model accurately forecasts physiological trajectories, inflammatory markers and patient outcomes. Simulation experiments indicate that combined plasma–red blood cell resuscitation outperforms crystalloid or plasma alone and suggest that earlier administration of plasma reduces injury severity and prolongs survival. Such “digital twin” approaches offer a translational bridge to optimise timing and composition of resuscitation in individual patients whilst informing the design of future clinical trials.
