Tremor Pathophysiology and Clinical Management

Summary

Tremor, an involuntary, rhythmic oscillation of body parts, arises from aberrant neural circuitry within the cerebello-thalamo-cortical network and related subcortical loops. Essential tremor, the most prevalent action tremor, is thought to involve Purkinje cell dysfunction, altered inhibitory GABAergic signalling and maladaptive oscillatory synchronisation between cerebellum, thalamus and motor cortex. Parkinsonian tremor reflects distinct basal ganglia and cerebello-cortical interactions, with characteristic rest-state oscillations. Clinical management encompasses pharmacotherapy—principally non-selective β-adrenoceptor antagonists and anticonvulsants—together with botulinum toxin for focal forms. In refractory cases, surgical and non-invasive neuromodulation techniques, including deep brain stimulation of the ventral intermediate nucleus and transcranial stimulation of cerebellar or cortical targets, can markedly reduce tremor amplitude. Recent advances in digital phenotyping and remote monitoring are improving fine-grained assessment of tremor frequency and stability, guiding personalised therapy. Emerging evidence of gut–brain interactions and microbiota-derived neurotransmitters further broadens understanding of tremor pathogenesis and opens novel therapeutic avenues.
