Trophoblast Cell Dynamics in Reproductive Health
Summary
Trophoblasts are specialised cells originating from the outer layer of the blastocyst that orchestrate implantation, placental formation and maternal–foetal exchange. Cytotrophoblasts proliferate and differentiate into syncytiotrophoblasts, which mediate nutrient and gas exchange, and extravillous trophoblasts (EVTs), which invade the maternal decidua and remodel spiral arteries. These processes depend upon tightly regulated programmes of proliferation, migration, invasion, apoptosis and differentiation. Alterations in trophoblast function underlie common obstetric complications such as recurrent miscarriage, pre-eclampsia and foetal growth restriction. Emerging evidence highlights the roles of metabolic intermediates, immune modulators, redox homeostasis, epigenetic modifications and non-coding RNAs in controlling trophoblast dynamics. Understanding these pathways offers prospects for predictive biomarkers and therapeutic intervention to improve maternal and perinatal outcomes globally.

Recent studies have uncovered a link between trophoblast metabolism and implantation success by profiling succinate dynamics. Reduced intracellular succinate, driven by altered succinate dehydrogenase promoter methylation, diminishes hypoxia-inducible factor signalling in EVTs, impairing invasion and correlating with recurrent spontaneous abortion in human samples and mouse models. Restoration of succinate levels in early pregnancy may offer a metabolic strategy to support placentation.
Investigations into immune-metabolic regulation have highlighted indoleamine 2,3-dioxygenase (IDO) as a key regulator of trophoblast proliferation and migration via the STAT3–MMP9 axis. Lower placental IDO activity is observed in unexplained recurrent spontaneous abortion, while IDO overexpression enhances trophoblast motility through STAT3 phosphorylation. Modulation of this pathway could inform new approaches to manage immune-mediated pregnancy loss.

Succinate dehydrogenase (SDH): A mitochondrial enzyme complex that oxidises succinate to fumarate in the tricarboxylic acid cycle and contributes to electron transport.
Indoleamine 2,3-dioxygenase (IDO): An enzyme that catalyses the first step in tryptophan catabolism along the kynurenine pathway, modulating immune tolerance and cell proliferation.
