Trophoblast Cell Invasion Mechanisms in Pregnancy
Summary
Trophoblast invasion is a pivotal process in early pregnancy by which specialised placental cells migrate into the maternal decidua and remodel uterine vasculature to establish adequate blood flow to the developing foetus. Cytotrophoblasts differentiate into an invasive extravillous lineage that acquires mesenchymal characteristics through an epithelial-to-mesenchymal transition-like programme, expressing proteolytic enzymes and adhesion molecules that enable penetration of the spiral arteries. This finely tuned invasion is governed by growth factor signalling networks, notably members of the transforming growth factor-β superfamily, as well as integrin-mediated adhesion, matrix metalloproteinase activity and cytokine modulation of the local immune milieu. Disruption of these pathways can lead to insufficient arterial remodelling, contributing to disorders such as pre-eclampsia, foetal growth restriction and preterm birth. Advances in stem-cell models, three-dimensional organoid cultures and single-cell transcriptomics have begun to unravel cell-type-specific programmes and identify key regulators of the invasive trophoblast phenotype, offering new avenues for intervention in placental dysfunction and therapeutic targeting of adverse pregnancy outcomes.
