Ulinastatin-Based Immunomodulatory Strategies in Acute Lung Injury and Sepsis
Summary
Ulinastatin, a broad-spectrum urinary trypsin inhibitor, has emerged as a promising adjunct in the management of acute lung injury (ALI) and sepsis. By modulating protease activity and attenuating exaggerated inflammatory cascades, ulinastatin protects endothelial and epithelial barriers, mitigates cytokine-driven tissue damage, and promotes resolution of systemic inflammatory responses. Preclinical models have demonstrated that ulinastatin preserves pulmonary tight junction integrity, reduces neutrophil infiltration, and limits oxidative stress in the injured lung. In patients with sepsis, ulinastatin administration is associated with lower organ dysfunction scores, improved oxygenation indices, and reduced incidence of multiple organ dysfunction syndrome. Mechanistically, ulinastatin down-regulates key pro-inflammatory mediators such as tumour necrosis factor-α and interleukin-6, inhibits nuclear factor-κB and mitogen-activated protein kinase pathways, and fosters the release of anti-inflammatory cytokines. These multifaceted effects underpin its ability to restore immune homeostasis, decrease capillary leakage, and ultimately improve clinical outcomes in critically ill cohorts.

Recent observational analyses have firmed the case for ulinastatin in sepsis management. A large retrospective cohort study evaluated the impact of adjunctive ulinastatin on 28-day mortality in critically ill septic patients. Use of ulinastatin was linked to a substantial reduction in mortality risk, with mediation analysis indicating that suppression of acute-phase reactants—principally C-reactive protein—accounted for over one-third of the observed survival benefit. Economic evaluation within the same cohort showed higher drug and total hospital costs in the ulinastatin group, yet adjusted models preserved the mortality advantage. This work underscores both the immunomodulatory power of ulinastatin and the importance of balancing therapeutic gains against resource implications in intensive care settings.
