Summary
Urethral duplication is a rare congenital malformation characterised by the presence of two distinct urethral channels. Incidence estimates vary but suggest fewer than one affected birth in 300 000, with a strong male predominance. The spectrum ranges from incomplete accessory tracts that present as prepubic sinuses to complete dual channels with separate bladder insertions. Embryological origins remain under investigation, with leading theories implicating aberrant cloacal septation or duplication of the urogenital sinus. The Effmann classification divides anomalies into three main types—partial accessory passages, complete secondary urethra originating from the bladder, and complex triplications. Clinical manifestations span asymptomatic findings to double urinary streams, urinary incontinence, recurrent infections and associated deformities such as penile curvature (chordee). Up to 70 % of cases carry concomitant anomalies including bladder or colonic duplication, vesicoureteric reflux, vertebral defects and renal dysplasia. Diagnostic evaluation relies on multimodal imaging—voiding cystourethrography, ultrasound and MRI—to delineate channel anatomy and plan surgery. Management is highly individualised, ranging from simple excision of an accessory tract to multi-stage urethroplasty using tissue flaps. Long-term outcomes generally show preserved continence and satisfactory cosmetic results when procedures are tailored to anatomical subtype. Global collaboration between paediatric urologists, radiologists and reconstructive surgeons is essential to refine classification, optimise interventions and improve patient-centred care.
