Valproate-Induced Hyperammonemia Management

Summary

Valproate-induced hyperammonemia is a serious but often under-recognised complication of valproic acid therapy, affecting patients receiving treatment for epilepsy, bipolar disorder and other neuropsychiatric conditions. The syndrome arises primarily from disruptions in hepatic and mitochondrial metabolism, leading to carnitine depletion, impaired β-oxidation of fatty acids and urea cycle dysfunction. Clinically, elevated ammonia levels may present with altered mental status, ataxia, confusion and, in severe cases, encephalopathy or coma despite normal liver function tests. Management strategies encompass both preventive and acute interventions. Preventive measures include identification of high-risk patients (young age, polypharmacy, strict vegetarian diet, pre-existing metabolic insufficiency), routine monitoring of plasma ammonia and carnitine levels, use of extended-release valproate formulations and prophylactic L-carnitine supplementation in susceptible individuals. Upon detection of hyperammonemia, immediate steps involve dose reduction or discontinuation of valproate, initiation of ammonia-lowering agents such as lactulose or rifaximin, and judicious use of L-carnitine to restore mitochondrial function. In severe or refractory cases, extracorporeal techniques—most commonly continuous renal replacement therapy—are employed to expedite ammonia removal. This multidisciplinary approach, integrating neurology, psychiatry and critical care, aims to reverse neurotoxic effects rapidly while preserving seizure and mood control through alternative regimens when necessary.

Valproate-Induced Hyperammonemia Management publication trend

The graph below shows the total number of articles in valproate-induced hyperammonemia management across all publications each year (not limited to Nature Index journals).

Technical terms

Valproate: Branched short-chain fatty acid used as an anticonvulsant and mood stabiliser.

Hyperammonemia: Elevated blood ammonia concentration causing neurotoxicity and encephalopathy.

L-carnitine: Endogenous compound transporting long-chain fatty acids into mitochondria; supplemented to counter valproate-induced depletion.

Lactulose: Non-absorbable disaccharide that acidifies colonic contents, reducing ammonia absorption and promoting its excretion.

Continuous renal replacement therapy (CRRT): Extracorporeal blood purification modality used to remove ammonia and other toxins in critically ill patients.

About these summaries

This Nature Research Intelligence Topic summary is created with the cited references and a large language model. We take care to ground generated text with facts, and have systems in place to gain human feedback on the overall quality of the process in line with our AI principles. We strive to create accurate and useful summaries for people unfamiliar with the research topic and that supports this goal. These pages are a beta release and will be updated as we learn how best to help people gain value from a research topic summary.
