Summary

Vestibular migraine is a neurological condition characterised by recurrent episodes of vertigo accompanied by migrainous features such as headache, photophobia and phonophobia. Diagnostic consensus criteria emphasise a history of migraine, vestibular symptoms lasting from minutes to days and exclusion of alternative causes. Clinical evaluation integrates patient history with physical examination and targeted vestibular testing—including the head impulse test, caloric stimulation and vestibular-evoked myogenic potentials—to rule out peripheral disorders. Neuroimaging may remain normal interictally but can reveal functional alterations in multisensory networks. Management encompasses acute symptom relief with antiemetics and triptans, alongside preventive strategies combining pharmacological prophylaxis—often adapted from migraine regimens—and vestibular rehabilitation. Lifestyle modifications, including dietary triggers, sleep hygiene and stress reduction, support long-term control. A tailored, multidisciplinary approach addresses the heterogeneity of presentations and aims to restore balance and quality of life.

Recent studies have revealed heightened sensitivity within central vestibular pathways in patients with vestibular migraine. Self-motion perceptual thresholds to roll tilt—a stimulus engaging both semicircular canals and otolith organs—were markedly lower in affected individuals than in controls, indicating sensitisation of canal-otolith integration. Two distinct patient subgroups emerged: one with threshold elevation correlating with symptom severity and vestibulo-ocular reflex dynamics, and another with uniformly low thresholds independent of clinical measures. These findings support a model in which migraine-related brainstem regions sensitise vestibular nuclei, while inhibitory feedback from the cerebellar nodulus modulates aberrant signals. This conceptual framework offers potential diagnostic biomarkers and targets for therapeutic intervention.
